When visiting our Passport Office, please ensure you have the following:

  • Completed DS-11 Form (in black ink only)

    • Note: We only process Form DS-11 (2022) applications.

  • Personal check, cashier’s check, or money order (for the passport fee, payable to "U.S. Department of State")

    • Note: Both a check/money-order and a credit/debit card are required for processing.

  • Credit or debit card (for the execution fee, payable to "Skyline College")

    • Note: Both a check/money order and a credit/debit card are required for processing.
  • Original and Photocopies of Proof of Citizenship (old passport, Certificate of Naturalization, or birth certificate)
    • Minor applicants will require a birth certificate for verification purposes even if they possess an old passport.

  • Government-issued photo identification (of the applicant or both parents if the applicant is under age 16) and a hard copy of both the front and back sides.

  • 2x2 color passport photo with a white background (photo service is available at our facility)

Children Under 16 Years Old

  • In addition to both parents supplying valid identification, the child’s birth certificate and social security number is required.

Children Aged 16 to 18 Years Old

  • May complete proccess on their own if they possess a valid California ID and provide a check in the parent’s name as proof of parental consent.

Two Required Payments:

Processing Fees - Skyline College Passport Office

  1. The following must be paid by credit/debit card to Skyline College
  2. Execution fee: $35 per application (Required)
  3. Express mail: $28.75 for overnight expedite mail service (1 to 2 day delivery) from Skyline College to Passport Processing Center. (Optional)
  4. Passport photo: $16 for two photos (Optional)

US Passport Book Fees - U.S. Department of State

  1. Must be check or money order made payable to "U.S. Department of State"
  2. One check or money order per application
  3. The Passport Card is an extra cost of $15 for minors and $30 for adults.

Please Note: As a Passport Acceptance Facility, we are authorized to receive your completed application for a U.S. Passport and submit the application along with all necessary documentation to the U.S. Department of State, Passport Services Division. It is important to note that only the U.S. Department of State can make the final decision to issue a passport to an individual.
