U.S. Passport Checklist 

When you visit our Passport Office, please make sure you have:

  1. Completed DS-11 Form in black ink only
  2. We only process Form DS‐11 (2022) application 
  3. Personal check, cashier’s check, or money order (passport fee to "U.S. Department of State")
  4. Credit/debit card (execution fee to "Skyline College")
  5. Old passport, Certificate of Naturalization, or original birth certificate (one of them and a Hard Copy)
  6. Government-issued photo identification of applicant or both parents if under age 16 and a Hard Copy of the front and back side
  7. 2x2 White background color passport photo (photo service available at our facility)

Children Under 16 Years Old

  1. In addition to both parents supplying valid identification, the child’s birth certificate and social security number is required

Two Required Payments:

Processing Fees - Skyline College Passport Office

  1. The following must be paid by credit/debit card to Skyline College
  2. Execution fee: $35 per application
  3. Express mail: $28.75 for overnight expedite mail service from Skyline College to Passport Processing Center
  4. Passport photo: $16 for two photos (optional)

US Passport Book Fees - U.S. Department of State

  1. Must be check or money order made payable to "U.S. Department of State"
  2. One check or money order per application
  3. The Passport Card is an extra cost of $15 for minors and $30 for adults.

Please Note: As a Passport Acceptance Facility, we are authorized to receive your completed application for a U.S. Passport and submit the application along with all necessary documentation to the U.S. Department of State, Passport Services Division. It is important to note that only the U.S. Department of State can make the final decision to issue a passport to an individual.
